Swiatek produced a stunning fightback to battle past Sabalenka in a gripping final that lasted over two hours.
The victory avenges Swiatek's loss in last year's final to Sabalenka and extends her winning streak to 13 matches.
Swiatek is now the first Polish woman to win the Madrid Open title.
